We will ask you to sign in, as well as ask for the prescription for physical therapy that you received from your doctor. We will also ask for and copy your insurance card if you are using insurance. The treating therapist will call you in for your initial evaluation. Expect to discuss the following with your therapist:

  • Your medical history.
  • Your current problems/complaints.
  • How/when your symptoms started.
  • The level, type and intensity of your pain, what aggravates and eases your symptoms.
  • How your symptoms are impacting your daily activities or limiting your functional activities.
  • Your goals with physical therapy. Medications, tests, and procedures related to your health.
  • The therapist will then perform an objective evaluation.
A plan for treatment and recovery is then developed with the your input. This includes how many times you should see the therapist per week, how many weeks you will need therapy, home programs, patient education, short-term/long-term goals, and what is expected after discharge from therapy. This plan is created with input from you, your therapist, and your doctor.
What should I wear for a Physical Therapy session at PARAGON?
You should wear loose fitting clothing that allows your therapist access to the area they will be evaluating and treating. For example, if you have a knee problem, it is best to wear shorts. For a shoulder problem, a tank top is a good choice, and for low back problems, wear a loose-fitting shirt and pants, again so we can perform a thorough examination and begin treatment.  WE do treat on the first visit.
Do You Accept My Insurance
In most cases, health insurance will cover your treatment. Click on our insurance link above for a summary of insurances we accept and make sure you talk to our receptionist so we can help you clarify your insurance coverage.

In some instances, we may be considered an “out of network” provider.  If your insurance has out of network benefits we may still be able to provide your treatment.  We also accept checks, or cash for payment of physical therapy services.

If you have additional questions regarding specific insurance issues, please call our office.
How Long Is Each Session
Expect to spend about 1 hour with your therapist on the first visit.  Depending on the complexity of your diagnosis and the plan developed for you, you can expect to spend 45 min to 1 hour each session.
Do I Need A Prescription?
In the state of New York your therapist can see you for up to 10 visits or 30 days without one.  After that you will need a prescription to continue with physical therapy. ** The exception is for workers compensation and no-fault cases. These programs REQUIRE that you have a prescription before we can see you. If you are coming to physical therapy under workers compensation or no-fault you MUST bring the prescription with you on your first visit.
