Does PARAGON Physical Therapy take insurance?

We work with various insurance plans, including Medicare. When setting up your appointment, please let our office staff know which insurance you have.  Since each insurance provider has its own requirements, we are happy to assist you in figuring out exactly what type of coverage you have and if your sessions qualify under your current plan.

How do I make an appointment?

We welcome you to give us a call to set up your appointment. Our staff will walk you through the simple process of becoming our patient, including any forms you may need to fill out.

What should I bring to my first appointment?
  • Your health insurance card and personal identification.
  • A physical therapy prescription from your health care provider, if needed.
  • A current list of your medications and supplements
  • Payment if you have a deductible or co-pay.
  • Completed forms. You can download and complete the necessary paperwork prior to you first visit here.
  • Please either wear or bring clothing that is comfortable and allows access to the area we will be treating (i.e. shorts or loose fitting pants and a comfortable T-shirt or tank top).
What should I expect during my first visit?

You will be welcomed by one of our office staff, who will collect your completed paperwork. To save time, you can download the forms here.

The initial sessions is about an hour long and consists of a thorough evaluation. During this session, the therapist will review your history and your symptoms, as well as, listen to your concerns and expectations for therapy.

Based on this initial evaluation, you and your physical therapist will outline the course of your therapy and options for treatment. Goals and expected time-frame will be set as well.

What happens when my insurance runs out?

If we both feel that you can benefit from ongoing therapy, we will work with you on developing a plan that is both effective and affordable.
